本研究提出一种双层规划模型,用于优化家庭姑息护理网络,上层最大化可及性与公平性等社会目标,下层最小化经济成本。模型引入考虑旅行时间、交通距离和患者年龄的新可及性函数,并采用模糊目标规划与模糊机会约束规划相结合的方法处理层次结构与参数不确定性。通过伊朗癌症控制中心的实际案例验证,该研究将家庭姑息护理纳入分散式规划,有效平衡了社会与经济优先事项。

Home palliative care enhances patients' quality of life by delivering compassionate care at home, reducing hospital visits and overall healthcare costs. This study proposes a novel bi-level programming model to optimize healthcare networks, specifically for home palliative care services. The upper level maximizes social objectives such as accessibility and equity, while the lower level minimizes economic costs. A new accessibility function is introduced, incorporating travel time, traffic distance, and patient age. Additionally, demand is estimated using patient characteristics and workload-based staffing indicators. To address the complexities of the hierarchical structure and uncertainty in parameters, the model uses a Fuzzy Goal Programming (FGP) approach integrated with Fuzzy Chance-Constrained Programming (FCCP). The model is validated through a real case study at the Iranian Cancer Control Center. This research contributes to healthcare logistics by integrating home palliative care into decentralized planning, effectively balancing social and economic priorities.
